Calabogie Sore Toe
I post this to calm anyone who is still very worried about my swollen big toe. According to Dr. Google, I have gout, malaria or am 3 months pregnant and retaining fluid. I am treating myself with all the remedies suggested - ginger, sweet cherries and peanut M&Ms (red are the most powerful).
Just to be ready for the worst case which would be the removal of the entire right foot, I have ordered a wooden foot as a replacement from the French Trading Co. of East Jesus, Nebraska. Should be here by next Monday in plenty of time should it be needed.

James we can't rule out the liklihood of all three conditions as you describe, the Interweb knows these things. In the event of the worse-case scenario we think maybe a lead-foot not wooden will keep you moving at the pace at which you've become accustomed.
